Home Assessments

Assessments for home care services and discharge planning for senior citizens and disabled adults.

Preadmission Screenings are provided to assist persons considering nursing home placement, assist in discharge planning and recommend alternative services as appropriate.

Alternative Community programs include Alternative Care Program, Elderly Waiver, and Community Access for Disability Inclusion and Traumatic Brain Injury Waiver. These programs are intended to promote community living and independence by providing appropriate health care and support services based on individual needs.

Crow Wing County Senior Advocate Program

Senior advocate program is an information and referral source for Senior Citizens in Crow Wing County. They provide seniors information concerning social, economic, financial and health-related services.

Advocates are available to talk with county seniors at area Senior Centers / Clubs or by phone. To contact your Senior LinkAge Line at 800-333-2433.
