You may request a certified birth certificate in person at our vital records counter. If you request the certificate in person, you will be asked to complete and sign an application (PDF) stating that you have tangible interest in the certificate. You will be asked to show identification.
To apply for a certified birth certificate by mail, please fill out the application (PDF) and have it notarized; enclose check or money order for $26, payable to Crow Wing County Land Services (additional copies are available for $19 each); mail to our office at the address listed to the right, and we’ll be happy to fulfill your request. We do not accept faxed requests at this time.
