Required Forms

If you are applying for a new On Sale and Sunday Liquor Sales license for your establishment complete the forms below and submit them to our office along with the required fee, listed.

• Crow Wing county Application for Liquor License (PDF) / Background Application
• State of Minnesota License Applicant Information (PDF)
• MN Workers’ Compensation - Certificate of Compliance (PDF)
• Retailer’s (Buyer) Card for Liquor Application (PDF)
• Liquor License Checklist (PDF)

All forms must be processed and a license issued propr to selling liquor.

Fees

• Total fee for new application for On Sale and Sunday Liquor Sales: $2250 
• Retailer’s (Buyer) Card for liquor or wine: $20 

Required Approvals

• Consent of Town Board: Contact the Town Clerk immediately to be placed on the agenda; town board approval on this form (PDF) is required prior to returning the application
• Upon receipt of all documents, our office will obtain the following approvals; please allow 6 to 8 weeks:
  • Sheriff
  • Attorney
  • Board of Commissioners
  • State Liquor Control Board
